Information Regarding Procedures by which Minors(under 20 years of age) (I)
Minors applying for resident visas
Required Documents
1. Household registration documents issued by a Taiwan regional administration within the last three months (this must have proof of the parent’s marriage).
2. Birth certificate, (original and one copy-the original will be returned after processing). This document must show the parents’ names; foreign documents must be authenticated by an ROC mission abroad).
3. Health Certificate issued within the last three months from a hospital designated by the Department of Health’s Center for Disease Control to provide health certificates to foreign nationals. Health Certificates from a qualified foreign hospital can be authenticated by an ROC embassy or overseas Mission; children under Six years of age are exempt from this requirement.
4. Passport (valid for more than six months and containing blank pages) and photocopies of the passport’s basic information page and page with entry stamp.
5. Visa application form (applicants must personally sign for confirmation; forms can be requested at the counter or downloaded on the Bureau of Consular Affairs’ website)
6. Two 2×1.5-inch color photographs taken within the last six months.
7. Other requested documents for certain individual cases.
